BEFORE YOUR APPOINTMENT
PATIENT SCREENING FORMS FOR ALL PATIENTS
- ALL PATIENTS -Will receive a booking confirmation email, and the day before your treatment, a reminder email and text.
- Your confirmation emails will contain a link to the relevant patient form you will need to fill out before your appointment, this link only needs to be filled out once before each appointment.
- All forms can easily be filled out on all devices. As soon as you click send your form will attach to your patient file and so no printing is necessary.
- Please do not transfer your link to a friend or family member if they are taking your appointment. This is a unique link that attaches to your personal file, they will need to book in to get their own link.
ALL NEW PATIENTS - Please fill out the New Patient consent form emailed to you.
ALL EXISTING PATIENTS - Please fill out your follow up form (emailed to you before every treatment).
NOT COMPUTER SAVVY? DO NOT WORRY...
- NEW PATIENTS – Paper New Patient forms will be available at the clinic.
- Please turn up to your appointment 20 minutes beforehand to give you enough time to fill out the paperwork to avoid eating into your appointment time.
FEELING UNWELL? PLEASE RESCHEDULE.
Be it a standard cold, cough or Covid we would much rather reschedule your appointment, keep our vulnerable patients safe and the clinic open. Although we have a 24-hour cancelation policy, we have modified our cancellation policy to offer greater flexibility to all our patients. Please let us know as soon as possible to allow time to fill your appointment.
MASK WEARING – IT’S YOUR DECISION, YOU HAVE OUR FULL SUPPORT
- If you would like us to wear a mask during your consultation, then please let us know and we will happily oblige.
- Please respectfully keep your distance from anyone in the clinic wearing a mask.
- Turning up to clinic unwell or snotty? Please respect our decision to keep the clinic safe and open if we decide to reschedule your appointment.
WHAT YOU NEED TO BRING AND/OR CONSIDER:
- LOOSE FITTING/COMFY CLOTHING - Always recommended.
- FACE MASK – If you choose to wear a mask.
- YOUR OWN WATER BOTTLE/THERMOS – Please bring some water with you, as it is important to hydrate post treatment.
DURING YOUR APPOINTMENT:
- PLEASE ARRIVE ON TIME - We need to run to time to thoroughly clean, ventilate each treatment room and avoid patient crossover.
- DO NOT BE LATE - If you are running late, please call the clinic, we may need to reschedule your appointment.
AFTER YOUR APPOINTMENT
PAYMENT CHARGE OPTIONS:
- We accept all usual card payments (sorry no American Express)
- We also accept telephone payments and so you can prepay over the phone or in person and we can type your payment in.
- You can also prepay by purchasing online treatment vouchers via our website.
CLINIC CLEANING PROCEDURES:
- Rest assured we have very proficient cleaning procedures. The clinic is checked yearly and certified by the Jersey Care Commission. We also LFT regularly.
